This project aims to increase early diagnosis and detection of respiratory conditions in the community and reduce emergency admissions and readmissions by increasing community expertise and empowering them to manage a larger proportion of respiratory patients, with specialists working alongside generalists.
The team at Sandwell and Birmingham Hospitals NHS Trust recognise the need to change the way they work in order to deliver high quality care, and the new ‘Right Care, Right Here’ programme places emphasis on teams working with no barriers. Striving to show how care should be provided to patients, the changes anticipate the opening of a new hospital, the Midland Metropolitan Hospital (MMH), in 2018.
